Eligibility

In order to use most city programs and ADA paratransit, you must complete an application certifying your eligibility. There is one consolidated application for all city programs. For ADA paratransit, please fill out the application for service in your area.

The web-based form for the city program application is currently unavailable.

While we update the form, please download and complete the PDF application, then mail it directly to the program you are applying for. Visit the Communities page to find your city and access its mailing address and application information.
